As members of the Pennsylvania Culture Change Coalition we are...

...the caregivers, the providers, the regulators, the families, and the consumers of elder care services in Pennsylvania. We strive to create deep system change toward person-centered care and to break down barriers to the redefinition of quality of life in our society.

Our Vision, Mission and Passion

• We articulate a vision of aging that is life-affirming, satisfying, and meaningful.
• We advocate for transformation across all long term care settings from an institutional model of care to a relationship and community focused model of living.
• We facilitate dialogue among all who care for and about elders in Pennsylvania.
• We believe this change will enhance the care of all including the sick and the disabled of all ages.
• We create and support networking and learning as the cornerstones of change.
• We believe that, like life itself, this change is not a destination but a journey.
